Understanding the three stages of labour

  Stage One: Labour

  This stage itself unfolds in phases. Early labour brings mild, irregular contractions as the cervix begins to soften and dilate, and can last several hours. Active labour follows, with stronger contractions roughly every 3–4 minutes as the cervix dilates from about 4 to 7 centimetres. Transition, the final and most intense phase, takes the cervix from 7 to the full 10 centimetres needed for delivery. This whole stage typically takes 8 to 20 hours, often longer for first-time mothers.

  Stage Two: Delivery of the Baby

  Once the cervix is fully dilated, active pushing begins, guiding the baby through the birth canal. This stage typically takes 2 to 3 hours.

  Stage Three: Delivery of the Placenta

  The final stage, delivering the placenta, usually happens within a short time after the baby is born.

Signs labour may be starting

• Lightening — the sensation of the baby settling lower into the pelvis.
• Bloody show — a thick, pinkish mucus discharge as the cervix softens.
• True contractions — regular, strengthening contractions that don't stop with rest or position changes.
• Water breaking — a gush or trickle of fluid, which should prompt calling your doctor right away.

Pain relief options

Labour pain is common, and there are options to manage it, discussed as part of your birth plan:

  Epidural analgesia

  Often called “painless labour,” this numbs the lower body through an injection in the back, while keeping you awake and able to push.

  Natural methods

  Controlled breathing, hydrotherapy, and continuous support can all help manage pain without medication.

Is normal delivery right for you?

• Fetal position — a head-down (cephalic) position is ideal for a safe vaginal delivery.
• Maternal health — the absence of complications like placenta previa or severe pre-eclampsia supports a normal delivery.
• Baby's estimated weight — assessed alongside your specific pelvic capacity, not as a fixed cutoff on its own.
• Gestational age — 37 weeks is considered early-term, with 39–41 weeks generally optimal for full-term delivery.

This is always an individual assessment, made together with you as your pregnancy progresses — not a decision made in advance without your specific circumstances.

Benefits of normal delivery

• Generally faster recovery than surgical delivery.
• Lower risk of infection.
• Often a shorter hospital stay.
• An easier start to breastfeeding for many mothers.

Recovery after normal delivery

Most mothers are ready to go home within a day or two. Full recovery, including healing of the pelvic muscles and vaginal area, generally takes about 4 to 6 weeks — postnatal care and guidance continue throughout this period.
